| Fire dept evacuation plan approved | Date the fire department approved the facility’s evacuation plan |
| Landline phone available 24/7 | Whether a landline is accessible at all hours |
| Emergency numbers posted | Whether emergency contact numbers are posted in required locations |
| Mobility-impaired residents | Whether any current residents have a mobility impairment (triggers additional facility requirements) |
